We are a Tiered-Supports agency with the Missouri Department of Mental Health. Tiered-Supports use a “tools of choice” approach for positive behavioral change and a better quality of life.
The mission of Perfect Partners is to support the daily life of individuals to help them become active participants in their communities, learn job and life skills, develop hobbies and interests, and create positive relationships.
We believe these are key components to a happy and healthy life. We treat our clients with acceptance, dignity, understanding, and respect. We ensure that the individuals we work with obtain their rights to be involved in and make choices for their daily lives and planning for their futures.
Our Story
In 2013, we opened our doors at Perfect Partners so that we could become an independent living support provider. With personal roots in the town of West Plains, Missouri, we felt like this was a great place for us to help others.
From modest beginnings, we have grown from a group of four to a staff of fifty employees. Our team has a wide range of experience that includes providing care in hospitals, psychiatric units, and small practices.
For the individuals we serve, we offer them a rich dailiy life, community integration, and fun social events and recreational opportunites they might normally not be able to participate in.
Over the last five years, we have been honored to provide services to individuals from all across the state of Missouri. We enjoy watching our clients learn and grow. As much as we enrich their lives, we are certain they have enriched our lives even more.

A lifetime Missouri and West Plains resident, Jacob has found his calling in the home health industry and by helping others in his community and across the state.
Chuck Julius
Owner
Chuck grew up in West Plains and attended Missouri State before going to trade school in Tulsa, Oklahoma. For the next fourteen years, he worked in managerial positions for several large corporations. Wanting to settle down and make a difference in his community, Chuck took on his current role at Perfect Partners.
Tia LaFevres
Manager
Tia has been with Perfect Partners since its inception. After a neice passed away from brittle bone disease, Tia was inspired to work in healthcare. She grew up in West Plains.
